Exciting opportunity to join a team of highly motivated and energized professionals. Must be able to function at a high level with little oversight on a Monday - Friday day shift schedule. Coordinate and provide nursing care, training, education and home support for patients. Work collaboratively and develop relationships with team members, patients and skilled nursing facility representatives. Collect, analyze and record (EMR) relevant patient health data and develop patient care plans.

Job Details

Qualifications
BS or AA/diploma in Nursing
At least 18 months of RN experience in hemodialysis needed
Valid license in TX
Current BLS certification
Driver's license required. Must be able to travel to local area sites.
Must be able to lift 50 pounds.
